Remote Case Manager RN - Michigan

Job Summary: We're looking for a talented Case Manager RN to work remotely in Southern Michigan, with occasional travel to the Detroit area (up to 15% of the time). As a key member of our team, you'll be responsible for assessing, planning, implementing, and coordinating comprehensive case management activities to promote the overall wellness of our members. Your expertise will focus on evaluating medical needs, facilitating care planning, and collaborating with healthcare providers to ensure optimal outcomes. Key Responsibilities:
- Conduct comprehensive assessments of members' needs and develop personalized care plans to address their unique requirements
- Utilize advanced clinical judgment and critical thinking skills to facilitate effective care coordination and collaboration with healthcare providers
- Apply clinical expertise to identify and mitigate risk factors, address complex clinical indicators, and promote optimal health outcomes
- Provide crisis intervention, support, and follow-up to members experiencing behavioral health or medical crises
- Collaborate with multidisciplinary teams to overcome barriers, present cases, and achieve optimal outcomes
- Identify and escalate quality of care issues, influencing medical and behavioral health professionals to ensure exceptional member care
- Empower members to make informed healthcare decisions, promoting lifestyle and behavior changes to achieve optimal health
- Analyze utilization, self-report, and clinical data to inform care planning and drive positive health outcomes
Requirements:
- Current, active, and unrestricted RN licensure in the state of Michigan
- 4+ years of clinical experience as an RN, with 1+ year of case management experience
- Willingness to travel up to 15% of the time within the Detroit area and surrounding counties
- Reliable transportation and residency in one of the specified counties in Michigan
- Bachelor's degree in Nursing required
Preferred Qualifications:
- Experience using Microsoft Office products, including Word, Excel, Teams, and Outlook
- Complex disease management experience, ranging from pediatrics to adults
